From Models to Decisions

Using exposure, simulation, and modeling to inform real-world decisions

Learn how pharmacometric models translate into decisions through exposure–response analysis, simulation, and model-informed drug development.

Welcome

Up to this point, you’ve learned how to:

  • understand pharmacokinetics and variability
  • build and estimate models
  • evaluate whether those models are reliable

Now we answer the most important question:

How do we use models to make decisions?

This module connects modeling to real-world applications in drug development and clinical pharmacology.

Why this module matters

Pharmacometrics is not about building models for their own sake.

It is about using models to:

  • select doses
  • design clinical trials
  • assess risk and benefit
  • support regulatory decisions

A model has value only if it informs a decision.

Warning

A technically correct model that does not change a decision has limited value.

Learning objectives

By the end of this module, you will be able to:

  • Explain exposure–response relationships
  • Understand how simulation supports decision-making
  • Interpret model outputs in a decision context
  • Describe the role of Model-Informed Drug Development (MIDD)

Course structure

This module follows a progression from relationship → prediction → decision:

  1. Exposure–Response Thinking
    • Linking PK to clinical outcomes
    • Why exposure matters more than dose
  2. Simulation and Scenario Thinking
    • Exploring “what-if” scenarios
    • Predicting outcomes under different conditions
  3. Model-Informed Drug Development (MIDD)
    • Using models to guide decisions across development
    • Connecting analysis to real-world impact

Key idea

All pharmacometric workflows ultimately aim to answer:

What should we do next?

Models help us:

  • understand current data
  • predict future outcomes
  • quantify uncertainty

But the goal is always:

  • better decisions

What you’ll be able to do after this module

  • Connect exposure to clinical outcomes
  • Use models to simulate alternative scenarios
  • Understand how modeling informs dosing and trial design
  • Interpret results in a decision-making framework

How this connects to the broader course

This module represents the culmination of the course:

  • PK → what happens to the drug
  • Models → how we describe it
  • Evaluation → whether we trust it
  • Decisions → what we do with it
